THIS IS NOT A SOLICITATION ANNOUNCEMENT. THIS IS A REQUEST FOR INFORMATION ONLY. This Request for Information (RFI) is intended for information and planning purposes only at this time; and shall not be construed as a solicitation or as an obligation on the part of the Department of Veterans Affairs. Because this is a Request for Information announcement, no evaluation letters and/or results will be issued to the respondents. No solicitation exists. Therefore, do not request a copy of a solicitation. See Instructions on page 6. Humphrey Field Analyzer 3 (Brand Name or Equal) INTRODUCTION/BACKGROUND: A visual field test is an eye examination that can detect dysfunction in central and peripheral vision which may be caused by various medical conditions such as glaucoma, stroke, pituitary disease, brain tumors or other neurological deficits. Visual field testing can be performed clinically by keeping the subject's gaze fixed while presenting objects at various places within their visual field. Simple manual equipment can be used such as in the tangent screen test or the Amsler grid. When dedicated machinery is used it is called a perimeter. OBJECTIVE: The Coatesville VA Medical Center Optometry, (592) Coatesville, Pennsylvania, hereby referred to as the VAMC Coatesville (592), is seeking the acquisition of one (1) Humphrey Field Analyzer 3 with Liquid Trail Lens, Printer & Table or equivalent. MAJOR REQUIREMENTS: The Contractor/ Vendor must complete the following tasks to meet the needs of this acquisition: A.1 INTRODUCTION This contract is for the delivery, installation of standalone instrument, and training of a (1) Humphrey Field Analyzer 3 with Liquid Trail Lens, Printer & Table or equivalent to meet the needs of Coatesville VA Medical Center Optometry. A.2 BACKGROUND A visual field test is an eye examination that can detect dysfunction in central and peripheral vision which may be caused by various medical conditions such as glaucoma, stroke, pituitary disease, brain tumors or other neurological deficits. When dedicated machinery is used it is called a perimeter A.3 SCOPE OF WORK One (1) Humphrey Field Analyzer 3 with Liquid Trail Lens, Printer & Table or equivalent will be delivered to the Coatesville VA Medical Center 1400 Blackhorse Hill Rd. Delivery shall include onsite installation and training. STATEMENT OF WORK PART B WORK REQUIREMENTS The vendor: Will provide delivery, installation and training to the Optometry Dept Coatesville VAMC and delivery that will be coordinated between the vendor and Coatesville VA Medical Center. The one (1) Humphrey Field Analyzer 3 with Liquid Trail Lens, Printer & Table or equivalent must have the following specifications: B1.2.1 Liquid Lens technology Using Liquid pressure, 1 trail lens for patients, using Liquid pressure, the new Liquid Trial Lens instantly delivers each patient s refractive correction, reduces patient setup time by automatically loading each patients refractive correction from previous exam. The result is faster patient flow and reduced chance of error by not needing to select a lens during set up, one trial lens for all patients. B1.2.2 SmartTouch interface: Touch screen interface allows for a more efficient workflow B1.2.3 RelEye: The RelEYE fixation monitoring system allows for systematic and pin point fixation inspection. The HFA 3 captures and keeps the image of the eye during the testing and just after, prior to the Result Review Screen. RelEYE cannot be viewed on the HFA 3 after terminating the test. Using the HFA 3, RelEYE is also available in FORUM Glaucoma Workplace (minimum version 2.0.3). Only FORUM can keep the RelEYE data stored for future usage for exporting exams. B1.2.4 Kinetic testing emulates standard Goldmann perimetry B1.2.5 Guided Progression Analysis (GPA ): GPA allows the doctor to compare visual field status and progression to data collected from glaucoma subjects. GPA provides statistically significant change as well as rate of change, and predict future vision quality. B1.2.6 RelEye: The RelEYE fixation monitoring system allows for systematic and pin point fixation inspection. The HFA 3 captures and keeps the image of the eye during the testing and just after, prior to the Result Review Screen. RelEYE cannot be viewed on the HFA 3 after terminating the test. Using the HFA 3, RelEYE is also available in FORUM Glaucoma Workplace (minimum version 2.0.3). Only FORUM can keep the RelEYE data stored for future usage for exporting exams. B1.2.7 Visual Field Index (Summary measurement of visual field status expressed as a percent of a normal age-adjusted visual field) B1.2.8 SITA-SWAP Blue-Yellow Perimetry: Replaces the Full Threshold and FastPac strategies for testing SWAP visual fields due to the much shorter test times available when utilizing SITA SWAP B1.2.9 Operating System: Windows 10 B1.2.10 STATPAC : It is a software package using normative values and patient data to perform statistically analysis to help interpret test data. STATPAC is the software, normative data and statistical analysis in HFA. B1.2.11 SITA (Swedish Interactive Threshold Algorithm) Unique patented algorithm utilized in the majority of case studies by universities and drug manufacturers, Normative data for SITA Standard and SITA Fast for 30-2, 24-2 and 10-2 threshold visual field test results from healthy subjects, ages 17 to 89, Central archive storage, combined reporting, seamless integration with EMRs, Dramatically reduces test time without sacrificing accuracy, Achieves the fastest possible test with less fatigue, Designed to deliver reliable result. B1.2.12 Glaucoma Hemfield Test Compares points in the superior and inferior hemifield to provide analysis of test results. On 24-2 and 30-2 tests, the GHT evaluates five zones in the superior field and compares these zones to their mirrored zones in the inferior field. The GHT evaluates the severity of disturbed points in each zone pair, relative to its normal database B2.3.13 The database can be removed from the instrument. B2.3.14 Specialty test library: Social Security Disability, monocular, binocular, Esterman monocular, binocular, superior 36, 64 Kinetic testing, Custom Kinetic testing, custom static testing B2.3.15 Complete visual field history efficiently presented in a single display, Serial testing presented on a single page report. Capable of Combine Report with Cirrus OCT. B2.3.16 General testing features- Stimulus sizes, Foveal threshold testing, Automatic pupil measurement, Liquid Trial Lens (AutoTLC), eye review B2.3.17 Software features- Single Field Analysis (SFA), Glaucoma Hemifield Test (GHT), Visual Field Index (VFI), Guided Progression Analysis (GPA), Serial field overview, Networking, , DICOM Connectivity B2.3.18 Internal Storage: 500 Gb B2.3.19 Electrical Requirement: 100-120v- 50/60 Hz, 4.0A B2.3.20 Connectivity: For comprehensive connectivity the HFA 3 can be connected to FORUM (minimum version 3.2) with FORUM Glaucoma Workplace (minimum version 2.0.3). DICOM conformant as per the DICOM Conformant Statement, available at http://www.zeiss.com/meditec/en_us/products---solutions/ophthalmology-optometry/dicom.html. HFA 3 also supports common file folder sharing used by most Electronic Medical Record Systems (EMR). B2.3.21 1-year warranty STATEMENT OF WORK GENERAL REQUIREMENTS DELIVERY INSTALLATION OF STAND-ALONE INSTRUMENT ONLY (CUSTOMER IS SOLELY RESPONSIBLE FOR INTEGRATION AND CONNECTIVITY TO NETWORK, IF APPLICABLE AND DESIRED). MANUFACTURER CAN PROVIDE TECHNICAL SUPPORT VIA REMOTE TELEPHONE CONVERSATION AS IT RELATES TO APPROPRIATE NETWORK PORTS AND SETTINGS. TRAINING TWO OPERATORS MANUALS TWO SERVICE MANUALS C.1.3. Vendor responsible for unloading, handling, unpacking; clean up to dumpster provided by GC C.1.4. Vendor to schedule deliveries through VA who would in turn schedule with GC. At the time of these deliveries, most if not all products will have to go through the loading dock, so a schedule will be arranged for dock use. C.1.5. VA to obtain and maintain current certificates of insurance for each vendor C.1.6. Vendor responsible for protecting product after installation C.1.7. Standard work hours are Monday Friday, 7:00 AM 3:30 PM C.1.8. Contractor shall provide proof of insurance to COR before any work starts STATEMENT OF WORK PART D SUPPORTING INFORMATION D.1. Place of Performance Coatesville VA Medical Center (hereafter referred to as, The VA , VAMC , or HCC ) D.2. Period of Performance Period covers installation and verification/testing of operations to ensure the equipment operate as marketed. D.3. Special Considerations D.3.1. Contractor Furnished Materials and Services D.3.1.1. Equipment to transport equipment (e.g., dollies, pallet jacks, etc.) D.3.1.2. Tools necessary to finalize installation of equipment (e.g., installation of casters, setup of shelving) D.3.2. Government Furnished Materials and Services D.3.2.1. Elevator access, power, and as optimal an operating environment as can be reasonably achieved. D.3.3. Qualifications of Key Personnel Each party will determine the level of skills and adequate training for personnel supplied. D.3.4. Contractor s Statement of Release - In consideration of the modification agreed to herein as complete equitable adjustment, the Contractor hereby releases the Government from any and all liability under this contract for further equitable adjustments attributable to this modification. D.3.5. Coatesville VAMC will provide a secure area for operation of the purchased equipment. The Government and the Contractor understand and agree that the services to be delivered under this contract by the Contractor to the Government are non-personal services and the parties recognize and agree that no employer-employee or master-servant relationships exist or will exist under the contract between the Government and the Contractor s employees. Delivery confirmation of non-equipment/ commodities acquisitions is the responsibility of the receiving Facility and/ or Service.
